If you would prefer you can print them off and either scan them into us or bring them along with you to your first appointment.

Please carefully fill this out and shade in areas of complaint on the pain diagram. Then indicate whether you consent or not to us contacting your GP should we need to. Please be thorough as approximately one in 15 people that we see we need to refer back to their GP because of complicating health reasons. We need to make sure that it’s safe to treat you.

Baseline outcome questionnaire form. If we don’t know where we have been we don’t know where we are going. This gives us a better understanding of how your pain is affecting your quality of life. We do these at regular intervals to measure how you are improving.

Once the paperwork has been completed, you will have a consultation with the chiropractor to discuss your health-related problems, concerns, as well as potential treatment options.

In order to determine what your actual problem is, the chiropractor will ask you various questions related to your condition. Following this they will carry out an orthopaedic and neurological assessment to confirm their diagnosis.

Your specific condition may occasionally require us to take images; either MRI scans or x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. Scans are not always necessary and research has shown imaging is not necessary for certain conditions including most types of back pain.

Once finished with their examination the chiropractor will explain to you what is causing your pain and how they can help.

As long as it is appropriate, patients will generally be provided with their first treatment during this same visit. This may include spinal manipulation, mobilisations, traction, taping, dry needling (acupuncture) and soft tissue therapies.

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches. It is important to take an active role in your recovery and health as shown by an abundance of research. At Backworks we strive to give you the tools to look after yourself.
